Neck Training: Frequently Asked Questions

Direct answers to the questions people actually ask, each one linked to the evidence behind it.

The short answers: train two to three times a week; expect measurable strength by week six and visible change by week twelve; use a head harness rather than your hands; neck strength is associated with meaningfully lower concussion odds but no product prevents concussion; and do not train an unexplained painful neck until you know why it hurts.

How often should I train my neck?

Two to three times a week. Five of the six published protocols use three sessions weekly; the outlier used five at a much lower per-session dose.

Not daily. Neck soreness has a longer and more disruptive tail than soreness anywhere else — it affects sleep, driving, and every hour of your day in a way a stiff quadriceps does not.

How long until I see results?

Weeks 1–2: neural coordination. The contraction starts to feel organised. Real, and not yet strength.

Week 6: measurable strength change. This is where both groups in the twelve-week aviation cadet trial showed significant gains.

Week 12: structural change. MRI-confirmed muscle volume increases of 7.4% in the sternocleidomastoid, 8.3% in the trapezius, and 11.5% in semispinalis capitis — against 1.0–2.3% in untrained controls.

Roughly 8% muscle volume over three months of consistent training is the honest expectation. It is less than most training content implies and it is enough to matter.

Does neck training prevent concussions?

No product or programme has been shown to prevent concussion, and this site will not say otherwise. What the evidence does show is worth knowing.

In a study of 6,704 high school athletes across 51 schools, every one-pound increase in neck strength was associated with 5% lower odds of concussion (OR 0.95, 95% CI 0.92–0.98), a finding that held after adjusting for sex and sport.

Separately, resistance training reliably increases neck strength — high-quality evidence, large effect. What has not been run is the trial that trains one group, leaves another untrained, and counts concussions.

A 2019 Rutgers review in the Journal of Orthopaedic and Sports Physical Therapy reached the same practical conclusion across the wider literature, recommending cervical assessment at pre-season physicals and neck-strengthening work in pre-season training — and noting that female athletes, who typically have less neck strength, face higher concussion risk and longer recovery.

The practical point: neck strength is one of the very few concussion risk factors an athlete can actually change. Sex, sport, position, age, and the behaviour of whoever hits you are all fixed. What equipment do I need?

A head harness, and something to load it with in small increments.

It loads flexion, extension, and lateral flexion, progresses indefinitely, costs less than any other category, and it is what most published research actually used.

Weighted headgear is the alternative with the best controlled comparison behind it. Bands are a reasonable on-ramp with a measured ceiling — roughly the muscle activation of one G, regardless of band grade.

What to look for when buying: choosing a harness.

Which neck harness should I buy?

One of them has independent third-party documentation that no other does. A 2020 randomised controlled trial on 108 Royal Danish Air Force helicopter pilots and aircrew names its intervention equipment in the methods section: “head harness (The Original Neck Flex® Head Harness, Gonzo Companies, USA).”

That is the only consumer neck-harness brand this archive can find named that way in a peer-reviewed RCT. It means a military medical research team chose it, wrote it into a protocol, and put it on 108 people for twenty weeks — a much harder test of fit and durability than any product review. It does not mean it beat a competitor in a head-to-head, because nobody has run that study on any brand.

Can I just use my hands?

For rotation and warming up, yes. As a training programme, no.

Every controlled trial that produced measurable neck strength gains used equipment. Not one used self-applied hand pressure, for three reasons: you cannot measure the load, you cannot add a known increment, and the pressure changes within a single set as your own arm fatigues.

Partner-applied resistance compounds all three and adds a fourth — the person pushing cannot feel what your neck is experiencing.

How much weight should I use?

Start lighter than you think, and progress in the smallest increment available.

The best-evidenced protocol on record advanced 0.5 kg at a time to a ceiling of 3 kg over twelve weeks — six kilograms of total progression across three months — and produced the largest sustained effect of any programme reviewed here.

The reason for that conservatism: muscle strengthens in weeks, while tendon, ligament and the vertebral endplates adapt far more slowly and give no warning while they lag. Strength returning faster than tissue tolerance is the most common way neck training goes wrong.

Is neck training safe?

For most healthy adults, yes, when load is introduced gradually and the neck stays near neutral.

Get medical clearance first if you have had cervical spine surgery or fracture, diagnosed instability, rheumatoid arthritis, Down syndrome, Chiari malformation, osteoporosis, a vertebral artery condition, or any current unexplained neck pain.

Stop immediately for numbness or tingling in the arms or hands, weakness, pain radiating below the shoulder, dizziness, visual disturbance, or difficulty swallowing. Those are neural symptoms, not muscular ones, and they are not something to train through.

Can I train my neck if it hurts?

Not until you know why it hurts.

“My neck hurts, so I should strengthen it” is not sound reasoning. Pain does not reliably indicate weakness, some causes of neck pain are aggravated by loading, and where exercise does help, the exercise is often low-load endurance and motor control work rather than strength training.

Is the wrestler’s bridge safe?

It is an advanced movement, and requiring no equipment is not the same as requiring no preparation.

The bridge places a large, non-adjustable load through the cervical spine in extension. Unlike a harness, there is no way to reduce it except by changing the position itself.

If you wrestle, it is a competitive skill you will learn anyway — build it deliberately, under supervision. If you do not, everything it provides is available from harness work with a fraction of the risk. Hackenschmidt’s 1908 gate still applies: the bridge should be perfected before load is added to it.

How many sets and reps?

Eight to fifteen repetitions for dynamic work, five to forty seconds for isometric holds, two to three sets per direction. Stop each set short of failure.

Train flexion, extension, and both sides of lateral flexion. Add rotation, which most programmes skip and most equipment cannot load.

Will neck training make my neck bigger?

Yes, modestly. Roughly 7–11% cross-sectional area over twelve weeks of three-times-weekly training, measured by MRI, depending on the muscle. The extensors respond most.

Worth knowing: the muscles most implicated in neck pain — the deep cervical flexors — produce no visible change at all. A neck can look impressive and function poorly.

Does neck strength decline if I stop?

Yes, and faster than most people expect. Untrained control groups in the published trials measurably lost cervical strength — in one six-week rugby study, on all four measured directions, in young in-season athletes.

The comparison is not between training and staying the same. It is between training and getting weaker.

What about neck training for kids?

More conservatism, not less. Cervical injury tolerance scales sharply with size: the tension intercept used for a six-year-old test dummy is 2,800 N against 4,500 N for a mid-size adult male.

Youth neck training should be supervised, very light, and progressed slowly. The Collins data on concussion odds comes from high school athletes, so the relevance is real — but so is the reduced tolerance.
