How to Program Neck Training
Frequency, volume, intensity, and progression — with the complete published protocols, reproduced exactly as their authors wrote them.
Train the neck two to three times a week. Use eight to fifteen repetitions for dynamic work and five to forty seconds for isometric holds. Stop each set short of failure. Progress load in the smallest increment available — the best-performing published protocol advanced in half-kilogram steps to a ceiling of three kilograms over twelve weeks. Expect measurable strength change by week six and structural change by week twelve, and expect roughly an 8% muscle-volume increase rather than the transformation most training content implies.

These are the actual prescriptions from controlled studies, not a synthesis. Where a study did not publish a detail, the cell says so rather than filling the gap.

The most completely specified protocol in the literature
The rugby study publishes more detail than any of the military work, which tends to describe its programmes in general terms. This is the whole thing.
Exercises. Weighted head-harness isotonic extension. Weighted isometric flexion. Weighted head-harness isotonic lateral flexion, left and right.

Load selection. Autoregulated. Players adjusted the weight against a ten-point scale, targeting a rating of 8 on the first set, 9 on the second, and 9–10 on the final set.
Notice what the isometric progression does: hold times fall from forty seconds to five across the block while load rises. That is a deliberate shift from endurance toward maximal force, and it is the opposite of the intuitive “hold it longer each week.”

Results. Left lateral flexion improved significantly (p=0.02); extension and right lateral flexion likely improved (p=0.07). Between-group improvements were 13.5 kg for extension and about 13.8 kg for lateral flexion. The control group declined on every measure.
The best load progression on record
The aviation cadet trial produced the largest sustained effect of any protocol reviewed here, and its progression is strikingly conservative.
Weighted-helmet group: load advanced in 0.5 kg increments to a ceiling of 3 kg across twelve weeks. Load was increased only when the cadet completed the set “effortlessly with proper form” and rated the effort below “somewhat hard.”
Session structure: 3 minutes of deep cervical flexor activation, 20 minutes of neck training across all six directions, 2 minutes of stretching.
The comparison group used progressive elastic bands, advancing through Thera-Band colours from blue to gold on the same schedule.
Outcome. Both groups improved by week six. Only the weighted group kept improving. Endurance effect size for the weighted group rose from 0.84 at week six to 1.01 at week twelve; the band group fell from 0.68 to 0.30.

Six kilograms of total progression over twelve weeks, in half-kilogram steps, gated behind a subjective ease criterion. That is what the best-performing published neck programme looks like.
Periodisation, where it has been used
The twenty-week Danish military trial is the only protocol in this set long enough to require real periodisation. It used undulating, non-linear periodisation — sets ranging from 2 to 4 and repetitions from 12 to 20, varied across the block rather than progressing monotonically, explicitly “securing a progressive overload.”
Its exercise selection is worth reproducing because it is more complete than most:
- Conditioning: supine upper cervical flexion and extension; cervical rotation against mild resistance
- Neck: cervical extension; flexion forward and oblique; lateral flexion
- Shoulder: shrugs; reverse flyes
Note the inclusion of shoulder work. Every long-duration protocol in this literature trains the neck and shoulder girdle together rather than isolating the neck.

The uncomfortable finding about adherence
That same twenty-week study is the most useful cautionary result in the entire literature.
Only 10 of 34 participants — 29% — trained regularly across the intervention. In the intention-to-treat analysis the effect was modest. Among those who actually did the programme, cervical extension strength rose 6.0 Nm against the reference group’s 2.4 Nm decline, with a between-group significance of p=0.001 — roughly triple the headline effect.
The programme was not the limiting factor. Attendance was. This was a supervised military intervention delivered inside working hours to professional aircrew, and seven in ten did not do it consistently.
Any programming decision that trades adherence for theoretical optimality is almost certainly the wrong trade. A six-minute session you perform three times a week beats a twenty-five-minute session you perform twice a month, by a margin this study quantifies.
What to expect, and when
Weeks 1–2. Neural coordination. The contraction begins to feel organised rather than effortful. Real, and not yet strength.
Week 6. Measurable strength change. Both cadet groups showed significant gains by the six-week measurement, and this is where band-based progressions begin to stall.
Week 12. Structural change. MRI-confirmed muscle volume increases of 7.4% in the sternocleidomastoid, 8.3% in the trapezius, and 6.6% in the deep neck musculature, against 1.0–2.3% in controls.
Week 20. In the only study of that length, the trained group had gained cervical extension strength while controls had lost it.
Nothing in this literature supports dramatic transformation. Roughly 8% muscle volume over twelve weeks of consistent three-times-weekly training is the honest expectation, and it is enough to matter.
A sample structure
This is not from a study. It is a synthesis of what the studies converge on, offered as a starting structure rather than a prescription.
Frequency: 3 sessions per week, non-consecutive days.
Session: 2 minutes of deep flexor activation, 12–18 minutes of loaded work across flexion, extension, and both sides, 2 minutes of unloaded mobility.
Sets and reps: 2–3 sets of 10–15 repetitions for dynamic work; 2–3 sets of 10–30 seconds for isometric holds.
Effort: stop each set with two or three repetitions in reserve. Never train to failure.
Progression: add the smallest increment your equipment allows, only when the current load feels easy with good form. Half a kilogram is a good increment. If your equipment cannot add less than 2.5 kg, add repetitions instead until you can.
Every 6 weeks: reassess. If nothing has changed, the problem is more likely adherence or insufficient load than programme design.
Read the safety page before beginning, and note that none of these protocols loaded the neck at end range. Keep loaded work in the middle of your range.
